Clinical Pharmacy

This specialization covers the pharmacist's role in patient care rather than in dispensing: medication history and reconciliation, dose review and adjustment for renal and hepatic function, drug interaction and contraindication screening, therapeutic drug monitoring, adverse drug reaction detection and pharmacovigilance reporting, patient counselling and adherence, and participation in ward rounds. Evidence appraisal and the literature are working tools.
